The following is an explanation for filling out the first line of a typical schedule form.

Enter name of organization providing league: WSYSA, DISTRICT 1, HSA
Enter name of league being scheduled: Premier, North, Silver, Red
Enter: Boy, Girl, Male or Female as required
Enter the standard age format: U-13, U-8, U-19
The following is an explanation for filling out the second line of a typical schedule form.

Enter name and phone number of the person responsible for direct coach contact regarding game scores. This person will be the official scorekeeper of the league. If a cross league is used, scores will be kept separately but by the same scorekeeper.
Enter name and phone number of the person that gathers the league standings from the scorekeepers for distribution to the news media. This person will record standings, game results etc. , for the leagues of the organization. The number of people holding this will vary depending on program size.
Enter name and phone number of the that is responsible for the formation and conduct of the league.
Enter the latest revision number (i.e. NEW, A, B) for this league. League revisions should be kept to a minimum, and no redistribution should be made without a revision number change.
Enter the date of league form distribution or redistribution that corresponds to the 'REV NO.' entry.
The following is an explanation for filling out the team information section of a typical schedule form.

Enter the acronym of the association or club that is sponsoring this team. Both association and club acronyms may be entered here.
Enter the name under which the team is registered.
Enter the name of the coach of the team. Enter the coaches first name instead of initials if this information is available. Space has not been provided for the coaches address.
Enter the phone number of the coach. FIELD Enter the name of the home field of the team. If several fields have the same name (i.e. Evergreen) enter additional information to provide better definition. This information should not be considered completely accurate. Home coaches should verify field location and game time for the visiting coach prior to each game. The home coach should also provide the field address and driving instructions to the visiting coach.
Enter the time when games are scheduled.
Defines team strength (i.e. Gold, Silver, U-13, U-14) in mixed, touch, and cross leagues.
