Mary at Comfy Bummy Diapers sells genuine Chinese prefolds (the best!) in a preemie size. $11.50/dozen.
The Preemie Store sells flat diapers (they look like prefolds) and nylon pull-on pants. Follow the link to "Feed me...Change me".
Alice at Preemie Sew Cute sells a flat (non-prefold) fold-to-fit 100% cotton flannel preemie diaper on her web site.
Barbara sells what she calls "Tiny Diapers" at B.Coole Designs. These run $20/doz.
Jennifer at Little Bundles will custom-make diapers in any size you wish. She makes prefolds in any size you wish (she recommends 14" x 14" from personal experience). She will also make fitted diapers. Please contact her for price.

For fitted diapers
Kushies Ultras - all-in-ones - more expensive, but very good quality. Available through Internet Retailers. They also sell other (lesser) quality diapers for different needs, including flat diapers.
The All-Together Deluxe All-in-One diaper comes in a preemie (4-7 lb) size for $5.50 each. These have elastic at the legs and a snap-tape closure at the waist to assure a good fit.
Robyn at Duzins Diapers will custom-make preemie diapers for you, using either the same wonderful plastic snaps used on Mother-Ease diapers, or hook-and-loop tape, your choice. (Right now she only sells through retailers - see her 'order' page - but you can write her via a link on her page for special size requests, and to get a price.)

Nicole, creator of Tiny Hineys will make fitted preemie-sized diapers upon request. She makes the tummy panel on the fitted's a bit wide, to protect from velcro fold-over rubbing on little bellies. All the elastic is hidden on the inside of the diaper, none touches baby's skin, and all the edges are turned in, no serged edges. The inner part of the diaper is only flannel, no stitching or elastic bunching on the inner "baby side" of the diaper. Available in choice of boy or girl prints. Diaper Wraps
Mary at Comfy Bummy sells preemie-sized Bummis Super Whisper Wraps
Jessica at Polar Babies (www.diapercovers.com) makes a polar-fleece wrap in Newborn size which she thinks will fit a baby as small as 4 lbs (though it's listed for 6-10 lbs). Fleece wraps are soft, non-abrasive, breathable and waterproof.

For disposables
Huggies - also available through some local retailers such as Baby Depot and Babies R Us
Pampers - enter your zip code on this page to be shown a list of retailers who will deliver to your home. Also, Babies R Us and Toys R Us may carry them - call your local store.
Tushies - the only commercially-available gel-free disposable diaper, available in preemie (1-3 lb) and newborn (4-5 lb) size. However, they will only sell by the case lot, which is 270 diapers per case. You may want to check with local health-food stores or co-ops to see if they can order this specialty size.
Gently Used Diapers in Preemie Size:
These are harder to come by, but not impossible. Check with Jenny at A Bottom Line - she sells an assortment of cloth diapers and covers in all sorts of sizes, including sometimes preemie sizes. She's very prompt to answer emails and very accommodating, in my experience. Her prices seem very fair, too.
Don't forget to check out the MOTShop - another buy/sell/trade web site for diapers and all sorts of mom-stuff. She gets in new consignments all the time.
Also, try posting requests on Amity's Cloth Diaper Sale and Swap Board, Angel's Baby Company Swap Board, Kanggahroo Creations' buy/sell/trade board for children's clothes and cloth diapers and Baby Essentials Sell/Swap Shop. Or, just go directly to www.insidetheweb.com and do a search for 'diaper swap'. You can't tell what kind of diapers people have stacked up in their house, no longer used since their kids outgrew them. Most of these boards permit other sorts of advertising, so be prepared to weed through it.
